Who is responsible
Phoenix Physiotherapy is responsible for personal information submitted through this website. Privacy and grievance enquiries may be addressed to Jayanta Pathak at phoenixphysio.info@gmail.com.
Information you provide
Appointment requests collect your name, contact details, clinic and scheduling preferences, and an optional short reason for visiting. Careers applications collect contact and employment information and a PDF résumé. Patient records, clinical documents, identity documents and financial information must not be submitted through this website.
How information is used
We use submitted information to coordinate appointment requests, communicate with you, evaluate employment applications, operate and secure the website, prevent misuse and meet applicable legal obligations. Phoenix Physiotherapy does not sell personal information or use it for behavioural advertising.
Children and guardians
An appointment request for a person under 18 must be submitted by their parent or lawful guardian. Children should not submit information or career applications directly through this website.
Retention and restricted access
Appointment-request information is retained for up to two years. Career applications are retained for up to 180 days and then automatically redacted unless an authorized reviewer deletes them sooner. Résumés are stored privately and restricted to authorized recruitment users. Access is limited according to staff responsibilities.
Service providers and cookies
The website uses Hostinger for hosting and Google/Gmail services for operational email. Embedded maps or links may connect you to Google services under their own privacy terms. At launch, Phoenix uses only cookies and similar storage needed for website operation, security and essential functionality; it does not use advertising or behavioural-tracking cookies.
Your choices and rights
You may ask to access, correct or delete information you submitted, withdraw consent where applicable, or raise a grievance by emailing phoenixphysio.info@gmail.com. We may need reasonable information to verify and process the request. Withdrawing consent does not affect processing already lawfully completed.
Security and updates
Phoenix applies role-based access, protected storage, audit records and reasonable technical safeguards. No internet service can guarantee absolute security. We may update this notice when the website, providers or legal requirements change.
